80 year old male patient came to opd with the c/o of shortness of breath
History of present illness
Patient has developed sudden onset of shortness of breath (grade lV) (he stopped  taking bronchodilators 2 days back ) brought to hospital
History of past illness
No H/O DM
No H/O HTN
No H/O TB
No H/O epilepsy 
Personal history
Married
Occupation:weaver
Appetite:normal
Bowel and bladder movements:regular
Allergies-no
Micturition- normal
Physical examination
Pallor-present
Pulse rate-117/min
BP-220/110 mm/Hg
Respiration-36/min
Systemic examination
CVS-S1 S2 positive
Thrills-absent
Cardiac murmurs-absent

Salt restriction
Inj.Lasix 40mg iv/bd
Tab.Nodosis 500mg po/bd
Tab.Bio D3 0.25mg po/bd
Tab.orofer -xt po/bd
Inj.Erythropoietin 400 iv/sc once weekly

60 year old male patient , who works as a daily wage worker hailing from Palem presented to the hospital with- • reduced micturition and defecation since 2 months. • burning sensation on micturition since 2 months.
